Emergency Care Information
|
Online Verification/Update (OVU) - Enrolling parents can use this packet at the beginning or during the school year to update phone numbers, emergency contacts, and health conditions for their child. Complete this packet to verify that FCPS has the most current information. To complete or update this packet, login to SIS ParentVue. Click the Online Packets tab in the top right-hand corner of the SIS ParentVue page. If you need a SIS Parent Account , please fill out a parent support request. PDF Fillable Form (English) |
| Health Information |
A health information form must be on file for all students. If you completed/updated the Online Verification/Update (OVU) your health information will automatically be updated. If you would like to complete a hard copy, send it via email to @email or mail/drop-off at school. PDF Fillable Form (English) |

Student’s Rights and Responsibilities |
Students will review the Student's Rights and Responsibilities (SR&R) in the second week of school. The SR&R Booklet can also be found on the FCPS website. Parents should sign the SR&R form in SIS ParentVue. |
