FCPS Required Forms for All Students

Emergency Care Form	To complete this form, login to SIS ParentVue. Click on the weCare school link in the FCPS Links button If you need a SIS Parent Acc ount , please fill out a parent support request. PDF Fillable Form (English)
Health Information	A health information form must be on file for all students. If you have completed all sections of [email protected] your health information will automatically be updated. If you would like to complete a hard copy send it via email to @email or mail/drop-off at school. PDF Fillable Form (English)
Student’s Rights and Responsibilities	Students will review the Student's Rights and Responsibilities (SR&R) in the second week of school. The SR&R Booklet can also be found on the FCPS website. Parents should sign the SR&R form in SIS ParentVue.
Parent Consent for Digital Tools	Some of the digital resources your child may use this year require parental consent. Complete the Digital Tools Parental Consent Form. Directions: How to Fill Out the Digital Tools Permission Form

Student Registration Forms	Please submit a new registration form if you have moved, but still live in South Lakes boundary. For more information about what’s required to change your address visit https://www.fcps.edu/registration/general-registration-requirements/documents

FCPS Privacy and Opt-Out Forms	The 2022-2023 Annual Notice of Survey, Records, Curriculum, Privacy, and Related Rights and Opt-Out Forms. These forms should be printed out and turned in ONLY IF YOU WISH YOUR CHILD TO BE OPTED-OUT.
Family Life Education Opt-Out Forms	Information about the FLE programs can be found on the FCPS website. Opt-out forms for all grade levels are there as well.
